  1. MicroMD PM - (Patient Demographics / Appointment Scheduling / Billing software) 
    1. Practice Administrators/Supervisors maintain this setup
    2. Provider setup requires a SIGNED form and is done by CPH
    3. Password reset is done by Supervisors/Super Users
  1. Dentrix - (Dental EHR)
    1. Practice Administrators/Supervisors maintain this setup
    2. Provider setup requires a SIGNED form and is done by CPH

    1. All new user requests must come into CPH by using the New EMR User form
